Eye/Dental/Physical exams are not required for entrance into Wisconsin Public Schools, however they are highly recommended. If you child sees their physician regularly for well-child check ups, your child's health needs should be met.

If you do not see a physician regularly for well-child check ups, you should make an appointment as soon as possible to ensure your child's vaccination requirements are met and to ensure that all medical concerns are addressed.


STUDENTS WITH MEDICAL CONDITIONS

Does your child wear glasses or hearing aides?
Does your child have a life-threatening allergy that they need an epi-pen for?
Require medication or specialized feedings/monitoring at school?
Does your child have a controlled or uncontrolled seizure disorder?
Any other medical condition they have been diagnosed with by a physician?
